This church is remembered since 982, under the name of San Leolino in Flacciano, in a parchment found in the Abbey of Passignano.
The present building dates from the twelfth century as can be deducted from the structure, but its appearance today is given by the restoration and transformation occurred in the sixteenth century.
The current building dates back to the 12th century as can be deduced from the structural and stylistic features. In medieval times the church was of great importance and 14 suffragan churches depended on it, including the one inside the Panzano Castle.
